Salicylic Acid vs Benzoyl Peroxide: Two Different Attacks on Acne
These two get treated as interchangeable "acne actives," but they attack the problem from opposite directions, and picking the wrong one for your specific breakout pattern is a common reason acne treatment stalls out.
Salicylic acid clears the blockage
Salicylic acid is oil-soluble, which lets it dissolve into the sebum-filled pore rather than being repelled by it the way water-soluble acids are. Inside the pore, it breaks down the cellular adhesion that causes dead skin cells to clump and block the follicle opening, which is the mechanical step that turns a normal pore into a blackhead, whitehead, or microcomedone. It is the right tool when congestion, blackheads, and texture are the main issue.
Benzoyl peroxide kills the bacteria directly
Benzoyl peroxide works through an entirely different, bactericidal mechanism: it breaks down into benzoic acid, which is metabolized in skin to release free-radical oxygen species that oxidize and destroy bacterial proteins in Cutibacterium acnes, the bacteria strongly associated with inflammatory acne.1 Because the mechanism is oxidative rather than chemical resistance-based, bacteria have not developed meaningful resistance to it, which remains a real clinical advantage over some antibiotic acne treatments.1
